It is important that you are participating within 7 days of the receipt date that is within seven days of your purchase.

Representatives of the company and their families can’t take part in this survey. With a good internet connection, you need at least one computer, smartphone, tablet, or laptop. You must be able to talk in English or Spanish to take this survey.
